Exposure to asbestos is the only known cause of this disease in the USA. Patients afflicted with Peritoneal Mesothelioma have inhaled or swallowed asbestos fibers usually through work related activities. Symptoms do not occur until 20 to 40 years after contact with the mineral and its by products. Malignant peritoneal mesothelioma is often fatal and patients who are afflicted by it live on average of less than a year from the time of their diagnosis.

Symptoms may include abdominal pain, abdominal swelling, a change in your bowel habits, such as more frequent diarrhea or constipation, lumps of tissue in the abdomen and unexplained weight loss.

Peritoneal Mesothelioma is first detected by X-rays or CT scans, peritoneoscopy (analysis of the peritoneum) or a biopsy where a tissue sample in taken for examination by a pathologist.

Treatment Options
Localized treatments treat only the original cancer site by either surgery or radiotherapy. A peritonectomy is a surgery used when peritoneal mesothelioma is detected early on. However, the symptoms are usually detected only in the most advanced stages because of the gradual development of symptoms. Complete removal of the caner through surgery is doubtful even in the earliest stages of the disease. The first 30 days following surgery are crucial. Unfortunately, the post surgery death rate for this operation is very high. Many patients do not make it through the procedure. With this in mind, many medical centers do not choose surgery as a viable treatment option plan at this time.

Regional chemotherapy is used in peritoneal mesothelioma cases, when the cancer is still localized at its point of origin. This option treatment is employed in both early and late stages of the disease using anti-cancer drugs. The drugs are injected directly into the abdomen. Patients undergo weekly or biweekly treatments depending upon the specific drugs used.

An additional procedure known as adjuvant chemotherapy, (regional chemotherapy) can be used directly following surgery in order to reduce the likelihood of the cancer returning. However, when the cancer cannot be entirely removed through surgery, chemotherapy will continue throughout the existence of the cancer in an attempt to slow its development.

A second type of treatment known as systemic treatments are used when the cancer has metastasized (spread throughout the body). Systematic treatments are implemented in either earlier stages or late stages of peritoneal mesothelioma.

Since peritoneal mesothelioma often remains dormant for years, the cancer is usually detected in its end stages. At this point patients are too ill to handle intensive chemotherapy. Doctors choose non-aggressive palliative care treatments at this time. Treatment goals are concentrated on remedying pain, discomfort and weight loss rather than attacking the disease head on. One type of pain common in the late states of this disease is caused by fluid buildup inside the abdomen. Doctors will attempt to reduce the pain and discomfort by draining the excess fluid through a procedure called abdominal paracentesis.

In benign pleural effusions we don't find a pleural cavity full of asbestos needles - It would appear we haven't been culturing for the right agent.

If Mesothelioma is a rogue viral infection - then there is the possibility of developing a vaccine and offering vaccination to asbestos exposed people who are at heightened risk of developing future asbestos related malignancies.

It involves structures like abdomen, stomach cavity, liver, spleen or the bowel. The origin of peritoneal mesothelioma is usually abdomen, from where it spreads to adjoining organs. The symptoms of peritoneal mesothelioma are:

Pain or swelling in the abdomen due to a build-up of fluid
Nausea
Weight loss
Bowel obstruction
Anemia or swelling of the feet
Abdominal bloating due to fluid accumulation in the abdominal cavity
Swelling of the feet
Fever
Impaired bowel function

The symptoms are a consequence of pressure exerted by the growing mass. This growing tumor can also cause breathing problems, and chest pain in addition to these symptoms. These symptoms result from tumors that lean against the abdominal wall. Mesothelioma in peritoneum, owing to its space occupying tendency may give rise to bowel obstruction, anemia, fever and problems with blood clotting.

Pericardial mesothelioma

Pericardial mesothelioma is a very uncommon form affecting the pericardium. Pericardium is the cavity between the linings of the heart. The tumor that grows in the tissue surrounding heart tends to spread from there. Mesothelioma is a rare and aggressive form of cancer. In most cases, the prognosis is very poor at the time of diagnosis. In general, patients have not more than twelve months to live following a positive mesothelioma diagnosis. Hence, the treatment options for such an aggressive and rapidly developing disease are quite limited. Mesothelioma treatment is usually palliative. In other words, the objective of treatment is simply to relieve the patient of the pain associated with the mesothelioma symptoms. There is no hope of curing the patient from the disease for several reasons:

(i) Diagnosis occurs very late in the development of the disease. By the time diagnosis is made there is no hope for the patient.

(ii) The development of the disease is very rapid and affects key organs like the heart and the lungs.

Surgery

Surgery or cytoreduction involves the removal of all or nearly all visible tumor. In cases of mesothelioma, this is generally combined with chemotherapy to give a more effective elimination of cancerous cells. Some surgical operations aim to cure the patient completely especially if the cancer is still localized. In most other cases, surgery may only have a palliative effect as the cancerous mass is reduced. The following types of surgical procedures are used in mesothelioma treatment:

(i) Pleurodesis: insertion of an irritant in the pleural space causing an inflammation; this closes down the pleural space thus preventing build-up of fluid (pleural effusion). Most symptoms associated with pleural mesothelioma can be abated with this method for some time. A thoracoscope is used for this.

(ii) Pleurectomy or peritonectomy: removal of part of the chest lining or abdomen lining (depending on where the cancer is).

(iii) Decortication: removal of all or part of the membrane covering an organ.

(iv) Pneumonectomy: removal of the whole affected lung (in most cases, mesothelioma develops on only one lung so that removal of that lung may cure the patient if the cancer has not spread).

Chemotherapy

Chemotherapy involves the administration of drugs that destroy cancerous cells. Chemotherapy in the treatment of mesothelioma can have the following objectives:

(i) Shrinking of tumors prior to surgery (neo-adjuvant chemotherapy).

(ii) Destruction of cancer cells after surgery (adjuvant chemotherapy).

(iii) Increase the effectiveness of radiotherapy (immunotherapy).

Chemotherapy is also used in cases where the cancer cells have spread beyond the initial site of occurrence. It is also used where the patient is not a candidate for surgery.

The most used drug for mesothelioma is pemetrexed which is an inhibitor of numerous proteins that are needed for DNA synthesis and cell replication.

Chemotherapy has various side effects because in the process of destroying cancer cells healthy cells are also harmed.

Radiotherapy

This refers to the treatment of cancer through the use of penetrating beams of high energy. In the case of mesothelioma, radiotherapy can be used in combination with surgery in an attempt to cure or to control severity of symptoms. An example of radiotherapy is the Intensity Modulated Radiation Therapy (IMRT) which uses computer generated images to target cancer cells directly with limited effect on surrounding tissue.

Every year, about 3000 new cases of mesothelioma are diagnosed in the United States alone. Unfortunately, mesothelioma is an aggressive and usually fatal type of cancer that results from asbestos exposure. This disease occurs when microscopic asbestos fibers are inhaled and get lodged into the linings of the lungs.

What are the chances for mesothelioma survival once a victim gets diagnosed? Sadly, not many victims of mesothelioma survive more than five years after the diagnosis has been made. Mesothelioma survival chances are increased if the disease is caught early. However, this kind of cancer is difficult to detect in its early stages. Once it is detected and diagnosed, it is oftentimes in its advanced stage and too late for a definitive cure. According to reliable sources, less than 10% of mesothelioma sufferers get the chance to survive more than 5 years, while about 40% of sufferers survive more than a year after diagnosis.

Mesothelioma Treatment Options

As in all forms of cancer, mesothelioma survival rates depend on various factors. These include:

Mesothelioma Survival - What Are the Chances?

* The current stage of the cancer. The stages of mesothelioma range from Stage One, which primarily means that the tumor is localized in the chest lining, to Stage Four, meaning that the tumor has spread to other vital organs.

* The histology (cell structure and type) of the cancer. Mesothelioma cancer can come as epithelial, which has better prognosis, sarcomatoid, biphasic or desmoplastic.

* The overall health condition and age of the sufferer. The malignant forms of mesothelioma are oftentimes diagnosed in sufferers above the age of 55. Generally, the older the victim is, or the more chronic illnesses associated with the disease, the less chance there is for mesothelioma survival.

* The size and location of the tumor (whether the tumor is operable or not.)

* The extent of the symptoms, such as whether fluid buildup has been observed in the lungs or abdomen.

Mesothelioma is a serious condition which demands immediate medical attention. Although there is no proven cure for mesothelioma, various treatment options are available to prevent the disease from spreading and also to reduce pain and suffering for the victim.

These treatment options may increase the chances for mesothelioma survival. According to various studies, victims administered with chemotherapy, radiation, or surgery, or a combination of any two or three, have a survival rate of at least 18 months after undertaking the procedures.
